A heart made with an arm
The recognizable detail is the space inside the shape. The upper arm, bent wrist and opposite hand must read together; a normal two-hand heart at chest height misses the joke. The close framing leaves room for the raised elbow, and the finished shape stays visible long enough to recognize.
